The MiraMar Loyalty Members Plan is not insurance and is not a qualified health plan under the Affordable Care Act. Savings are calculated on our standard professional fees. Enrollment requires a current exam at our practice. Membership is valid for 12 months from the date of enrollment. 30-day money-back guarantee: full refund within 30 days of purchase if no services or discounts have been used. Payment for treatment not included in the plan is due at the time of service. Discounts apply only to procedures completed at our practice by our providers; they do not apply to cosmetic treatments (such as bonding and veneers) or to orthodontics (including comprehensive orthodontic treatment and appliance therapy).
